In light of recent news and public discussions regarding proposed shifts in the organ donation landscape, most recently with HHS, ProCure On-Demand continues to stand firm in its commitment to operational excellence and clinical integrity. While three key challenges facing the field were addressed, we’re proud to share that these concerns are not new to us—they’re part of the very foundation of our daily work. This is not a pivot, but a confirmation: ProCure’s NRP processes were thoughtfully created to address these issues head-on, with solutions that are evidence-based, scalable, and deeply embedded in our mission. Here’s how we continue to lead with clarity, consistency, and care

1. Patient Safety & Public Trust: Respecting Donors and Families in the NRP Space

Patient safety and public trust aren’t just priorities, they’re principles. In the context of Normothermic Regional Perfusion (NRP), we uphold the highest standards of ethical and clinical integrity to honor donors and their families. Our approach is grounded in transparency, anatomical precision, and unwavering respect.

  • No Paralytics Ever - We do not administer paralytics in our perfusion prime under any circumstance. This is a deliberate and values-driven decision rooted in respect for the donor and clarity for the family. It reinforces our stance that NRP is a restorative process, not a manipulative one. Through our thoughtful approach and technique, the avoidance of paralytics has never negatively affected organ evaluation or recovery; we are able to ensure the Dead Donor Rule is maintained with full clarity and precision.

  • Thorough Review of Donor Anatomy - Every case begins with a meticulous evaluation of the donor’s aortic anatomy and arch anatomy to ensure procedural appropriateness and safeguard donor dignity. This step is the clinical backbone of our commitment to safety.

  • Respectful Communication with All Involved - Our teams prioritize time for thorough debriefs, open dialogue, and compassionate listening prior, during, and after a case. Pre-OR huddles and OR timeouts ensure all teams involved have a chance to connect and set expectations, offering a time to address any outstanding questions or concerns prior to a case beginning. Our job doesn’t end when we leave the OR. We follow-up to share case data and monitor organ performance where applicable. This isn’t just good practice; it’s the ProCure standard.

2. Maximizing Quantity & Quality: Reducing Discarded Organs, Elevating Outcomes

ProCure’s mission is simple and bold: to increase both the quantity and quality of organs available for transplant. In the context of NRP, we’ve built a process that delivers on this promise with precision and consistency.

  • NRP Initiation: Industry Leading 99.9% Success Rate - Getting on NRP is the first and most critical step in maximizing organ recovery. Our teams achieve this nearly every time, because we’ve built systems that prioritize readiness, coordination, and clinical excellence.

  • Full-Length NRP Commitment: Industry Leading 100% of the Time - We stay on NRP for the full duration required to appropriately assess and evaluate each organ. Regardless of which client or organ ProCure is there on behalf of, we are committed to staying on NRP to assess each organ.

  • Thorough Organ Evaluation Protocols - Our teams bring their own echo probes to assess cardiac viability, run serial labs to evaluate liver function, measure urine output for kidney perfusion, and optimize lung strategies through routine venting, hemoconcentration, and volume management. Every organ gets the attention it deserves.

  • Team Familiarity Drives Better Outcomes - We deploy NRP professionals (NRP/recovery surgeons, perfusionists, assistants) as a team; they work together often, ensuring a consistent approach and protocol. Literature supports what we see every day: familiar teams deliver better results. Our cross-collaborative training programs ensure that every member is aligned, prepared, and respected.

  • Minimizing Blood Transfusions for Organ Integrity - We actively minimize transfusions; a practice linked to improved organ quality and reduced inflammatory response. Compared to the industry standard of 6-8 units of pRBCs, we use 3 units or less on 92% of cases, on average giving 1.2 units of pRBCs per case.

3. Lowering Cost, Expanding Access: Making Organ Donation More Possible

Cost should never be the barrier to saving lives. At ProCure, we believe that increasing access to organ donation means rethinking the economics of recovery without sacrificing quality, safety, or respect. Our model is designed to reduce unnecessary spending while amplifying clinical value.

  • Missed Opportunities = Increased Cost - Every case where NRP isn’t initiated—or isn’t sustained—represents lost organs, lost lives, and increased financial burden for OPOs and transplant centers. We treat every opportunity as sacred, and every missed one as preventable.

  • Scalable, Replicable Process - ProCure’s NRP model is a true end-to-end solution. We bring everything: trained teams, equipment, medications, and evaluation devices. Our OPO and hospital partners simply provide the OR. There are no upfront costs or hidden fees—just risk-aligned pricing that reflects outcomes, not overhead.

  • Operational Efficiency Without Clinical Compromise - We streamline logistics, reduce redundancies, and optimize resource allocation to keep costs down. Every dollar saved is a dollar redirected toward expanding donation opportunities.

  • Minimizing Transfusions for Cost and Care - Blood is expensive—and precious. By transfusing on average fewer than 2 units per NRP case, we save thousands of dollars while preserving blood products for other patients and donors in need. The financial savings across a year of donation is profound; in the hundreds of thousands of dollars. This is possible due to our meticulous techniques and attention to perfusion detail.

  • Mission-Driven Cost Awareness - Our commitment to increasing the quantity and quality of transplantable organs includes making donation financially sustainable.
